Route 23 was an on-ramp located in White Plains, New York where a deadly crash occurred in Final Destination 2 on May 13, 2000.

Plot

Officer Burke was driving in his patrol car when a truck carrying heavy, thick logs had a chain that was loose and dragging along the road. After coffee spills on Burke's leg, the chain snaps, and the logs fall off the flatbed. Burke is too distracted by cleaning up the spill and, even though he hits the brakes, a log bursts through the windshield, obliterating him. Eugene Dix and Rory Peters see the accident and attempt to drive out of the way. Eugene's bike falls over and he is thrown against a log, where his motorcycle zooms across the road and slams into his torso.

In the midst of the pile-up, Kimberly's SUV crashes.

Rory hits a log and his vehicle starts to flip and roll out of control, finally landing in the middle of the road away from the confusion. Although Rory is not killed immediately, he looks to his left and sees a garbage truck headed right for his vehicle, which incinerates his car and kills him. Next, Kat Jennings hits a log in her SUV and it flips forward, landing on the roof, crushing her to death. Nora Carpenter and her son try to swerve out of the way, but she cannot hit the brakes because a water bottle has fallen and gotten lodged under them. She crashes her car into a log, exploding on impact. Kimberly Corman crashes and flips her SUV on a log and it rolls out of control, severely wounding her and her friends Shaina, Dano, and Frankie. Evan Lewis is futile in his attempts to avoid crashing into the gas tank of the truck carrying the logs and presumably dies.

A mysterious truck comes out of nowhere, obliterating Evan Lewis and heading straight towards Kimberly.

To Kimberly's surprise, Evan is still alive, and screaming as the fire burns his flesh away. A wounded Kimberly starts to cry, unable to save the man from his car, and suddenly a truck appears out of nowhere and annihilates him. The same truck drives straight for Kimberly's crashed vehicle with no attempts at stopping. Kimberly screams as the truck drives into her.
At that point, the vision ends, and Kimberly and her friends are still safe on the entrance ramp. Kimberly freaks out after signs in her premonition come true and she holds up traffic, as well as several of the people killed in her vision. Burke comes by and asks her what's wrong, but doesn't believe her until the truck carrying the logs passes by and the accident occurs. Burke retreats to call for help, but a moment later, he pulls Kimberly out of the way when the same truck that killed her in the premonition comes along, running into her SUV and killing Shaina, Dano and Frankie. Kimberly then looks at the crash and begins to cry.

Signs/Clues

• Kimberly sees a bus with people inside screaming "Pile up!", It was later revealed to be Mt. Abraham High School bus.
• A news announcement about the Flight 180 memorial plays on the radio.
• The song "Highway to Hell" plays on the radio.
• Kimberly sees a kid slamming a red toy truck against a bigger truck.
• Kimberly and Shaina see a man drinking beer in the Hice Pale Ale beer truck with the sign DRINK RESPONSIBLY! on it.
• Kimberly sees a sign stating NEXT 180 FEET.
• Eugene has a pool ball keychain with the unlucky number 13 on it.
• During the opening credits, there's a journal that says ROAD TRIP. However, a set of keys cover the T, spelling ROAD RIP.
• Before and after the premonition, a bum comes up to Kimberly's van shaking a bag of cans and the bag breaks releasing its cargo just like the log truck would.
• Kimberly's father, Michael see a gas leak from his daughter's SUV, appearing like blood.
• After Kimberly's premonition, when the real crash occurs, Officer Burke runs to his car, and speaks into his radio. He announces his unit is number 13.

The North Bay Bridge is the doomed crossway in the Final Destination timeline, before the explosion of Flight 180. Sam Lawton has a premonition of the bridge collapsing, killing him and everybody. His girlfriend, Molly Harper was the only person to survived from the disaster in Sam's original premonition.
The collapse occurs as the initiating disaster of Final Destination 

Cause of the bridge collapse

Authorities reported that the bridge collapse was started from high winds. Construction workers working on the bridge caused it to weaken as well.

What happened on the bridge

Sam and  Molly attempting to escape from the collapsing North Bay Bridge.

Many people flee Bus 1282 after the bridge starts to collapse, but Isaac stayed behind in the bathroom to have privacy to talk on his phone. Candice is trapped, and chunks of the road fall into the lake below. She hangs onto the railing of the bridge far off to the side, but the railing detaches, sending her to fall off the bridge and is impaled on the mast of a sailboat. Isaac comes out of the bathroom unaware of what has happened and unable to stop the bus which falls off the bridge. Isaac is thrown against the windshield, and crushed as water floods the bus.
The bridge then starts to collapse from the middle, forcing everyone to find an alternative route around. Sam helps Molly cross a beam that connects with the other side of the Bridge and she managed to get to the safe half of the bridge. Meanwhile, Olivia trips, and her

glasses are kicked into the water by nearby bystanders. Sam helps Olivia along the beam, but due to her poor eyesight, she is left helpless as she falls into the water below. She resurfaces, but is crushed by a car that had fallen off the bridge. Nathan tells Sam to wait up, but an out of control cable strikes Nathan, killing him. The bridge then starts to tilt right, forcing a car to slide and destroy a railing, leaving no grip for Dennis to hold on to, and he tumbles off of the bridge, but he grabs the edge of the bridge at the last second. He attempts to climb back up, but an explosion causes a tank to fall over, and spray Dennis with hot asphalt, melting his flesh, and killing him. Half of the bridge then starts to collapse, and Peter jumps onto a suspended guard rail. Sam gets a running start, and makes it as well. Finally, the other side bridge collapses completely. A truck then tips over, and multiple rebars fall and skewer Peter, and he dies and falls onto the concrete below. Molly, who's safe all along, screaming as a large metal sheet slides off the truck, bisecting Sam.

Sam wakes up from the premonition, and stops the bus from driving any further. He and Molly escape the bus, then Nathan follows. Peter also exits the bus, who is followed by Candice to make sure Peter is alright. Olivia calls after Molly, and follows everyone out. Isaac hangs up his call (which had previously caused his Death) and follows Olivia outside. Dennis then follows to know what is happening, just in time for the bridge to start collapsing. After the event, Sam and Molly embrace each other, while the other survivors look at Sam in horror. Death toll: 86

Signs/Clues

• The bus passes a sign that says "Danger: Construction", and a picture of a bridge. Part of the picture is scratched off, hinting that the bridge was going to collapse.
• Sam along with everyone else watch a video about corporate retreats. The name of the video starts with 180.
• The Logging truck on the road, is from a company called "Tagert". Tagert Theatres was a cinema that exploded in Nick O'Bannon's premonition 10 years later.
• The song Dust in the Wind is played on the bus.
• The bus number is 1282. 1 + 2 + 8 + 2 = 13, and 13 is an unlucky number.
• Sam sees a truck loaded with logs, a reference to Route 23 from Final Destination 2.
• In certain parts of the bridge the construction is on, there are a few visible cracks. A crack is then suddenly made by a worker's drill, meets with another crack, then starts the collapse.
• Before getting on the bus, Sam reads "Watch your step".
• While on the bridge, a worker holds up a sign which Sam reads "Slow".
• While crossing the bridge, a construction beacker falls off of the bridge, hits the concrete, and splashes into the water, forshadowing Peter's fate during the collapse.
• A dark shadow passes over Sam before he pricks his finger.
• The bus shakes due to the high winds, but only Sam feels and see's it.

Trivia

• The reason that this accident nor Sam and the survivors are ever mentioned again during the franchise is because all the people who knew about death's list had died. Everyone else, who wasn't associated with the list or investigating it, had believed this disaster to be a normal disaster. Everyone except Bludworth. In Final Destination, Flight 180 is the first known story brought to the public about premonitions and death's list. Same with all the other disasters as the survivors had told other people about what was going on, and they lived to tell the tale.
